From locating housing to reconciling medications to finding in-home care support, we can help get you back home. Direction Home offers two nationally recognized transition programs:
- Acute Care Transitions (ACT) – Designed for patients transitioning from hospital to home
- HOME Choice – Assists those moving from a long-term care facility (hospital, nursing home or residential treatment facility) to a community-based setting
