PASSPORT & Assisted Living Providers Title III & Care Coordination Providers Please Note: The PACE Program is not available in the 10B region. Referrals: Please note: Direction Home Akron Canton does not refer providers to consumers, instead, provider placement is based on consumer choice. REQUEST FOR PROPOSALS, Contract Cycle 2019-2020 Direction Home Akron Canton (Area Agency on Aging & Disabilities 10B) is soliciting proposals for its Title-III and Health Education programs to serve the 60+ population in Portage, Stark, Summit & Wayne Counties of Ohio for the period October 1, 2018 through September 30, 2020. Contracts are not automatically renewed; providers must submit a new proposal by the defined deadline to be eligible to participate in the next round of contracting. Following are the programs of Direction Home and the services in each program for which proposals are being accepted: Title-III B (click here for the proposal packet) Proposals will be accepted for the following services in Title-III-B: legal counseling; home maintenance & repair; and transportation serving the 60+ population. Proposals are due February 16, 2018. Title-III C (click here for the proposal packet) In Title-III-C proposals will be accepted for congregate and home-delivered meal programs serving the 60+ population. Proposals are due February 16, 2018. Health Education (click here for the proposal packet) Proposals will be accepted for evidence-based health education programs targeting the agd 60+ population, including A Matter of Balance (MOB); Chronic Diabetes Self-Management; Chronic Disease Self-Management; and Diabetes Empowerment Education Program (DEEP). The programs are health education and self-management programs held in a group setting, which provides activities and strategies to help persons improve their management of their own health issues. Proposals are due January 16, 2018. Care Coordination (click here for the proposal packet) Proposals will be accepted for the following services in Care Coordination: Adult Day Service; Chore; Emergency Response Systems; Home-Delivered Meals; Homemaker; Minor Home Modification; Personal Care; Social Work/Counseling; and Transportation, Medical.
