According to n4a "Direction Home Akron Canton has been identified as a leader in business acumen and partnering with health care." Due to this expertise, Mr. Cook and his team will participate in the yearlong effort that serves as a "think tank" for prototyping and collectively working towards solutions to address next generation obstacles and challenges to contracting with health care entities. Staff participated in the first of many sessions at n4a's national conference in Savannah, Georgia in July of this year. "We are honored to be included with these thought-leaders representing community based organizations across the country" said Mr. Cook. "Direction Home is known in the aging and disability network for sharing best practices and learning from others. We look forward to building on that history in collaborating with our colleagues to address the critical challenges presented by the growth of the older adult and disability populations and the need to efficiently allocate scarce resources. By working to develop partnerships with health systems and managed care organizations, we can work toward development of comprehensive systems of care that emphasize population health through integrated medical and supportive social systems that directly impact the social determinants of health." The funding for this opportunity is thanks to a grant from the U.S. Administration for Community Living to n4a. The purpose is to advance the readiness and business acumen of aging and disability community-based organizations (CBOs) to successfully engage in the design, development and delivery of integrated care systems that better integrate medical and social service supports for older adults and people with disabilities.
