Home / Resources / Service Providers / Current Provider Information / MyCare Ohio Provider Information MyCare Ohio-East Central MyCare Ohio is a three year demonstration project administered by the Ohio Department of Medicaid designed for individuals receiving both Medicare and Medicaid benefits into one program, operated by a Managed Care Plan. Statewide, MyCare Ohio will be available in seven regions covering 29 counties and approximately 114,000 individuals. The East Central region of the MyCare Ohio Demonstration project includes Portage, Stark, Summit, and Wayne Counties. For general MyCare Ohio information, click here. Required Participants: Eligible for Medicare (Parts A, B and D) and FULLY eligible for Medicaid Adults 18 and over with physical disabilities and persons over the age of 65 Must be living in one of the demonstration counties This includes: Individuals in nursing facilities, in some home care programs (PASSPORT, Choices, Ohio Home Care, Transitions Carve Out, Assisted Living Waiver) and who are receiving behavioral health services in community settings Exempt Participants: Individuals with an ICF-MR level of care served either in an ICF/IID facility or on a waiver administered by the Ohio Department of Developmental Disabilities Individuals who are eligible for Medicaid through a delayed spend-down Individuals who have credible third party insurance including retirement benefits Participant Enrollment: Consumers can enroll in or change their MyCare Ohio plan by visiting the Medicaid Consumer Hotline website or calling the Medicaid Consumer Hotline at 1-800-324-8680 If there are still questions once the consumer has contacted the Medicaid Hotline, the consumer can contact the Akron Canton Area Agency on Aging and Disability Resource Center (ADRC) at 1-877-770-555 East Central MyCare Ohio Plans: Two plans have contracted with the State of Ohio to serve Portage, Stark, Summit, and Wayne counties: CareSource and UnitedHealthcare Community Plan of Ohio Important contact information: CareSource Provider Relations Department firstname.lastname@example.org email@example.com 800-488-0134 For providers seeking contracts with the Managed Care Plans, following is contact information for staff in their respective contracting departments.
