Enrollees in this popular program receive personalized services that are coordinated by a care manager. The care manager can assist with things like disease management, health insurance benefits, service coordination and transitional care. Our professionally trained Care Coordination staff specializes in working with families who have loved ones with chronic conditions, such as heart disease, cancer or Alzheimer's disease.
