The program is administered by the Ohio Department of Medicaid. Direction Home Akron Canton works in partnership with CareSource and UnitedHealthcare Community Plan of Ohio to deliver the services available through the MyCare Ohio program. MyCare Ohio Benefits A Single Point of Contact: The MyCare Ohio program coordinates your Medicare and Medicaid benefits through one insurance plan. This will allow individuals to easily navigate their health care goals and needs across various services and health care settings. Person Centered Care: MyCare Ohio provides you with care focused around you and your needs. MyCare Ohio will provide you with choice and self-direction, support to remain independent and in the community, and care management that includes face-to-face visits in the home and community. This means that the management functions belong to the two health plans so we no longer have the authority to make certain decisions about individuals' care and services that we once had. MyCare Ohio Resources Medicaid Consumer Hotline (800) 324-8680 (800) 292-3572 MyCare Ohio Ombudsman: When consumers have concerns about their long-term care services - at home, in a nursing home, assisted living or other skilled care facility - they can turn to their local Long-term Care Ombudsman. Ombudsmen field complaints about long-term care services, voice clients' needs and concerns to nursing homes, home health agencies, and other providers of long-term care.
