Home / Services / Need Help Staying Home? / Ohio Home Care Waiver Services | Meal Sites/Senior Centers | PASSPORT | Assisted Living | Care Coordination | Ohio Home Care | MyCare Ohio | Long-Term Care Ombudsman | HOME Choice | Community-Based Care Transition Ohio Home Care Waiver The Ohio Home Care Waiver (OHCW) program benefit package consists of nursing services, personal care assistance services and/or skilled therapy services, plus waiver-specific services such as home modifications, home-delivered meals, adult day health care, out-of-home respite care, supplemental transportation, adaptive/assistive devices, and emergency response systems. CareSource contracts with Direction Home Akron Canton to provide case management services. Who is eligible? The OHCW program is designed to meet the needs of financially eligible consumers who have been assessed to require an intermediate or skilled level of care and who are age 59 or younger. Without the services available through the waiver benefit, these consumers are at risk for hospital or nursing home placement. Consumers approved for the OHCW benefit may receive care and services at home. Specialized Recovery Services Program The Specialized Recovery Services Program (SRS) is a Medicaid program for adults with serious and persistent mental illness. Although these individuals may have access to basic health services through Medicare or private insurance, Medicare and private insurance do not pay for service coordination and community support activities covered by Medicaid. How is eligibility determined? CareStar, the Council on Aging of Southwest Ohio, and CareSource will perform eligibility evaluations for the non-financial requirements of the SRS program and provide the recovery management services that support individuals receiving services. Individuals are also required to meet financial eligibility requirements, with determinations completed by local County Departments of Job & Family Services (CDJFS) offices.
