Are you or a loved one living with Alzheimer's Disease and/or Related Dementias (ADRD)?


Direction Home Aging and Disabilities Partners with Summit County Combined Health District Public Health (SCPH) for ADRD Services

Direction Home has partnered with several organizations to offer services and resources to Summit County adults living with Intellectual Developmental Disabilities, adults living with or at high risk for ADRD, adults experiencing behavioral management issues, and for the family caregivers of these individuals.

Dozens of older adults and their caregivers will benefit from this program through professional assessments performed by local subject matter experts. Benefits from the programming can include*:

  • Specialized home visits with licensed care professionals
  • Caregiver support, respite, and information

*for qualifying individuals


Direction Home continues to be there for those living with/at risk for ADRD, regardless of diagnosis, ability, or income by connecting individuals with available programs and opportunities.
Thriving at home is still an option for those living with ADRD, and their family caregivers!
