Direction Home Akron Canton Area Agency on Aging & Disabilities (DHAD) is soliciting proposals for The U.S. Administration of Community Living Older Americans Act Title-III B, Title-III C, Title-III D, Title-III E, and Care Coordination to serve the 60+ population in Portage, Stark, Summit, and Wayne Counties of Ohio for the period October 1, 2024 through September 30, 2026. 

Proposals will be accepted for the following services – Title-III B: home maintenance & repair, legal counseling, transportation. Title-III C: congregate and home-delivered meals.  Title-III D: health education - NCOA Highest Tier Evidence-Based Health Promotion/Disease Prevention Programs. Title-III E: caregiver support groups; caregiver education; and caregiver counseling. Care Coordination: adult day service and adult day service transportation; emergency response systems; home delivered meals; homemaker; home maintenance and chore; home modification; personal care; and transportation.

Can You Make a Referral for my Organization?

No. We do not refer any providers. Rather, provider placement is based solely on consumer choice. However, we will give our consumers and members all of the information they need to make the best choices they can for their long-term care, including details about our provider organizations.

In one year alone, our provider network helped older adults and individuals with disabilities to receive 300,000+ home-delivered meals, nearly 140,000 congregate meals, 848 transportation services, 420 home repairs… and so much more.
