Transitioning from an institutionalized setting, such as a hospital or nursing home, back to the comfort of your own home is a welcome change. However, it often comes with a number of challenges that can hinder your success and safety. Direction Home Akron Canton is deeply committed to helping our consumers achieve the best possible medical outcomes while remaining in community-based home settings. We want to help get you back home.

Direction Home also partners with community leaders like libraries, senior centers, educational groups, faith communities, first responder networks, and more to provide information, training, and other community-based initiatives to both professionals and community members.

If you or a loved one are preparing for a transition, contact the Aging and Disability Resource Center at 877.770.5558 for more information.

Mythbuster (4/8)
4. Myth: I will be pressured into applying/signing up for a program if I call the ADRC.
Fact: The ADRC will not try to sell you anything, and any decisions about long-term care planning will remain yours. The ADRC provides choices for individuals, and supports them in remaining independent at home. Calling the Resource Center does not obligate you to sign up for anything that isn’t the right fit for you (877-770-5558 / https://www.dhad.org/refer).
