Exciting news announced at the San Diego USAging conference: Direction Home won $100,000 for our submission to the ACL Health at Home Challenge! This Challenge is designed to support independence, improve health, and reduce costs for up to one million dually eligible (or near-dually eligible) Medicare and Medicaid beneficiaries by scaling advanced community care networks that partner with healthcare organizations to integrate healthcare and community care.
Direction Home leads a Community Care Hub model for all 12 Ohio Area Agencies on Aging to coordinate statewide aging networks and payer partnerships. The Community Care Hub helps streamline administrative, contracting, and operational oversight for community-based care.
