Assisted Living is a great option for many people wishing to retain a sense of community and self-sufficiency. The Assisted Living Medicaid Waiver program is designed to promote independence and improve physical and social well-being by providing participants with individualized care and assistance with housing, personal care and healthcare services.

Who Is Eligible?

To be eligible for Assisted Living you must:

  • Be 21 years of age or older
  • Meet the financial eligibility criteria for Medicaid determined by your County Department of Job and Family Services
  • Be able to pay a monthly room and board payment
  • Require hands-on assistance with at least two daily personal care activities

Community Transition Service Funding

There are designated funds available to enrollees leaving a nursing facility and relocating to an Assisted Living Facility. These funds are used to purchase household furnishing supplies and incidentals not provided by the facility, or formal/informal supports.
