Direction Home Akron Canton is passionate about the benefits of community-based living. That’s why we’re proud to be, through a contract with CareSource, a Care Management Agency for the Ohio Home Care Waiver (OHCW). OHCW allows individuals with physical disabilities and unstable medical conditions to receive care in their homes and communities instead of nursing facilities, hospitals or rehabilitation facilities. It empowers these individuals to have more control of their lives and remain active participants in their communities.

How OHCW Works

OHCW is a service that provides care for individuals from birth to 59 years of age. If you apply for the waiver, the Ohio Department of Medicaid will task us with conducting a level of care assessment of your needs. Once all aspects of eligibility are met, you will be enrolled in the waiver and a dedicated care manager will help develop a person-centered care plan to coordinate services.

Specialized Recovery Services Program

The Specialized Recovery Services (SRS) Program is another community-based service. A Medicaid program, SRS is for individuals aged 21 and older with serious and persistent mental illness (SPMI) or certain diagnosed chronic conditions (DCC). Direction Home Akron Canton works in partnership with CareSource to deliver services available through the program, including three services not available under a traditional Medicaid benefit plan:

  • Recovery Management
  • Individualized Placement and Support (Supported Employment)
  • Peer Recovery Support

There is no cost to apply and no co-payments or premiums for the SRS program. Once enrolled, the individual will keep their Medicaid benefits.
