If you’re an older adult, or if you’re caring for a loved one who is aging, you know that remaining independent in your own home is an important part of your well-being. PASSPORT is a Medicaid waiver program that provides older adults with the services needed to make the desire to stay at home a reality. From light housekeeping to accessibility modification to assistance with bathing, PASSPORT offers a comprehensive range of supports. PASSPORT services include:

  • Personal care aides
  • Adult day services (adult day care)
  • Emergency response units
  • Home delivered meals
  • Transportation
  • Minor home modifications
  • Chore
  • Specialty consults

Eligibility & Application

PASSPORT eligibility revolves around two main factors:

  • Physical help needed for everyday activities
  • Financial help to pay for in-home services

To enroll in PASSPORT or for more information and to determine your eligibility, you must first complete a phone screening to determine preliminary Medicaid eligibility and care needs. Once you are determined to be eligible, a care manager will work with you, your family and your doctor to develop a package of in-home services. The easiest way to get started is to contact the Aging and Disability Resource Center at 877.770.5558.
