Join Our Provider Network

Thanks for your interest in becoming part of our provider network! We ensure the highest quality of care services, and so our provider partnership comes with certain guidelines and regulations. Please note that Direction Home Akron Canton cannot supply referrals and/or consumers for start-up companies. Also be aware that prospective provider agencies are required to:

  • Currently operate in the community delivering services to consumers in home settings
  • Demonstrate compliance with the rules and regulations for each program
  • Have documentation demonstrating compliance with the elements of each rule
  • Be compliant with the regulations at the time of PASSPORT or Assisted Living Waiver application

Direction Home Akron Canton's Quality Improvement (QI) staff will verify compliance through an in-person visit to your facility. The rules and regulations are different for all programs, so we encourage you to familiarize yourself with the regulations before you submit your provider application.

Preparing for a Pre-Certification Visit

Once you have completed the application package through ODA and they have contacted us with the approval to move forward with your application, a QI staff member will get in touch to schedule a pre-certification visit at your office. During this visit, we will review your policies, procedures, personnel records, patient/client/resident records, and any other relevant documentation to verify your compliance with the ODA regulations. All relevant documentation should be available for review at the time of the visit. This visit typically takes 2-4 hours.
